Position: Registered Nurse - RN Case Manager - Hospice - FT - Dayshift

As a Hospice RN / RN Case Manager, you will:

• Report directly to the Administrator, Executive Director, or Patient Care Manager Senior, maintaining primary control of each patient’s care.
• Serve as the main liaison between physicians, patients, families, and the hospice team.
• Provide direct patient care and supportive services to families, managing a reasonable caseload with compassion and trust.
• Educate and empower caregivers on patient care needs.
• Collaborate with an interdisciplinary team including LPNs, CNAs, and support staff to ensure comfort, peace, and dignity.
• Assess patient and family needs, and develop and implement individualized care plans.
• Maintain accurate clinical records for all patients.
• Participate in quality assurance and performance improvement initiatives.

About You

Requirements for Hospice Registered Nurse / RN Case Manager:

• Compassionate, kind, and empathetic attitude towards patients and families.
• Comfortable providing care in a patient’s home environment.
• Flexible and composed in crisis situations.
• RN license in the state of employment or eligibility to obtain one.
• At least 1 year of nursing experience in hospice, home health, ICU, geriatrics, or oncology preferred.
• Additional experience in hospital settings such as Med Surg, ER, critical care, or wound care is a plus.
• Valid driver’s license and auto insurance.
• Current CPR certification.
