The Mary T Family of Companies has an immediate opening for a part-time Registered Nurse (RN) Case Manager for our Hospice department serving the western Twin Cities area and surrounding suburbs.

The RN Case Manager will be responsible for the overall case management of hospice clients and their families, and to plan and provide with the attending physician individualized nursing care and comprehensive service to patients, families and the community. For this position, the RN will need to be available to respond as needed while on call. This position is part-time, offering 20-30 hours per week.

Responsibilities Include

Assume overall responsibility for the coordination of the hospice patient's plan of care and ongoing evaluation of patient/family response to care

Initiates the plan of care on the initial visit, coordinating all patient/family services, and prioritizing patient/family needs with EDG members

Re-evaluates the patient's nursing needs and revisions to the plan of care as warranted with interdisciplinary team

Assist with Quality Assurance

Implements nursing policies and procedures relevant to nursing care

Maintains open line of communication regarding patient's status and any problems

Prepares clinical and progress notes, informing the physician and other relevant personnel of changes in the patient's condition or needs, and the outcomes of interventions

Determines scope and frequency of services, based on the acuity and patient/family needs

Provides specialized hospice training to other staff, family members and informal caregivers to ensure adequate care

Assesses the abilities of the caregiver to meet the patient's immediate needs upon admission and throughout care

Applies specific criteria for admission and re-certification to hospice care, to establish appropriate levels of care and patient's eligibility

Supervise the RN Home Visit Nurse and other caregivers

Schedule: Monday - Friday from 8:00 AM - 4:30 PM, 20-30 hours per week, including on-call duties every-other weekend.

Requirements

Minimum of an Associate's degree in Nursing required

Active RN License in Minnesota required

Minimum of 1 year nursing experience required, Hospice experience preferred

Must possess a valid driver's license with a good driving record and have reliable transportation

The ability to pass a background check

A willingness to work hard and a desire to help others

Strong verbal and written communication skills

Basic computer skills
