Job Description

Summary of Essential Functions:

As a Registered Nurse Case Manager (RN) at Committed to Caring, you will administer skilled nursing care to patients in their homes, following physician orders and the plan of care under supervision. You will participate in patient, caregiver, and family counseling and education, communicate with physicians and healthcare practitioners, and contribute to the QAPI program. You will develop and revise care plans in collaboration with patients and caregivers, supervise LVNs and HHAs, and ensure quality care delivery.

Qualifications:

  • Current RN license in practicing state(s)
  • Minimum 2 years' experience, home health preferred
  • CPR certification (American Heart Association or Red Cross)
  • Strong communication and interpersonal skills
  • Good customer service skills and commitment to quality
  • Valid driver's license, auto insurance, reliable transportation
  • On-call at least once a month; may require 100% travel

Educational Requirements:

  • High School Diploma or GED; graduate of an approved nursing program

Working Conditions & Physical Requirements:

  • Work indoors and in patient homes, with physical effort including lifting up to 100 pounds, kneeling, reaching, and exposure to environmental elements and biological hazards.

Shift: Monday-Friday, 8:30 AM - 5:00 PM

Coverage Area: Northeast & East Houston

Compensation & Benefits:
