Job summary

Trinity Health MI in Ann Arbor seeks an ED Case Manager to coordinate admission processes, ensuring patients receive the right care at the right time. This role reviews clinical appropriateness and communicates admission details to the Patient Access team.

You will work a 36-hour week on a rotating shift, with 12-hour shifts, weekend/holiday rotation, and a 6–8 week orientation. Benefits start Day One, with growth opportunities in the Trinity Health system.

Qualifications

• Current Michigan Nursing License.
• Associates degree in Nursing; BSN within 5 years of employment.
• Three years of recent case management/utilization review experience.
• CMSA certification desirable within three years.

Responsibilities

• Facilitates actions for admitted or emergency center patients to receive the right care at the right place at the right time.
• Assess readmissions; determine reasons and adjust admission plans; document in Epic CM.
• Identify discharge barriers; work with teams to resolve; report to Acute Care Flow Leadership.
• Consult with EC physicians to determine admission eligibility and status; document treatment plans.
• Ensure documentation aligns with hospital and insurance criteria for admission.
• Communicate medical necessity documentation requirements with attending physicians as needed.

• This position is a Full-Time, benefited position.

Miniumum Qualifications

• Current Michigan Nursing License
• Graduation from accredited nursing program: Minimum of Associates degree in Nursing, BSN required within 5 years from the date of employment in the role.
• Experience: Three (3) years recent experience in case management, utilization review or a related field.
• Certification: Case Management is desirable: Individuals serving as case managers for the health system shall obtain CMSA certification within three years from their date of employment in the role or be in the process of obtaining certification as a case manager.

What You Will Do

• Facilitates actions necessary for admitted or emergency center patients to receive the right care in the right place at the right time.
• Assess all patients that qualify as a readmission. Determine reason for return and implement alternative to admission plan when appropriate. Document readmission information/interventions in the Epic CM system.
• Identifies barriers to discharging patients from the E.D instead of admission as observation. Develops strategies and initiatives to address identified barriers. Work with other disciplines to assist in resolution of barriers to discharge. Report findings to Acute Care Flow Leadership team.
• Proactively consults with the EC physician to determine eligibility for admission. Facilitates appropriate documentation for inpatient admission as required by hospital and insurance criteria.
• Proactively consults with the EC physician to determine appropriate admission status (inpatient or observation) and level of care required. Facilitates appropriate documentation of reason for admission including the treatment plan as required by hospital and insurance criteria.
• Communicates medical necessity documentation requirements with attending physician as needed.
