Job summary

Help at Home is seeking an RN Case Manager in Indiana to support LaFayette and Logansport with weekly travel. This role offers a $5,000 sign-on bonus and a comprehensive benefits package including medical, dental, vision, 401(k) and PTO.

Qualifications

• Graduate of an accredited nursing program.
• One year of clinical nursing experience.
• Active Indiana RN license required.
• Experience with medically fragile clients and coordinating medical and social care.

Responsibilities

• Provides services in accordance with the plan of care.
• Performs initial evaluation and regular reevaluation of patient needs.
• Initiates and revises the plan of care as required.
• Conducts preventive and rehabilitative nursing procedures.
• Documents patient visits and care conference summaries promptly.
• Coordinates services and informs staff of changes in condition.
• Counsels patient and family to meet nursing needs.
• Processes orders, notifies physicians, and handles certifications/recertifications.
• Determines nursing amount needed per patient and supervises staff.
• Provides total patient care and participates in on-call duty as assigned.
• Completes and submits OASIS assessments and transfers per policy.

Responsibilities

What You’ll Do:

• Provides services in accordance with the plan of care.
• Makes the initial evaluation visit and regularly reevaluates the patient’s nursing needs.
• Initiates the plan of care and necessary revisions.
• Initiates appropriate preventive and rehabilitative nursing procedures.
• Prepares clinical and progress notes for each patient visit and summaries of care conferences on his/her patients in a timely manner as per Agency policy.
• Coordinates services.
• Informs personnel of changes in the condition and needs of the patient.
• Counsels the patient and family/significant others in meeting nursing and related needs.
• Processes orders and notifies physician of patient needs and changes in condition. Completes certification/recertification orders and discharge summaries.
• Determines the amount and type of nursing needed by each individual patient.
• Supervises and teaches other nursing personnel.
• Conducts patient care conferences on patients assigned to his/her care.
• Participates in peer review and Quality Assessment and Performance Improvement as assigned.
• Gives total patient care as needed; Takes on-call duty nights, weekends and holidays, as assigned.
• Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frames.
• Appropriately utilizes ICD-10 codes.
• Performs other related duties as assigned.

Education and Experience

• Graduate of an accredited nursing school.

Travel Requirements

• Regular travel on a weekly basis throughout an assigned territory.
