Job summary

Help at Home, the nation’s leading home care provider, seeks a Registered Nurse (RN) Case Manager dedicated to helping clients live safely and independently at home. This role supports LaFayette and Logansport with a focus on compassionate, comprehensive nursing care.

You will evaluate needs, initiate and adjust plans of care, coordinate services, and counsel patients and families. Regular travel in the territory is required, and a $5,000 sign-on bonus is available.

Qualifications

• Graduate of an accredited nursing school.
• One year of clinical nursing experience.
• Active Indiana RN license.
• Experience with medically fragile clients and addressing medical, social and behavioral care components.

Responsibilities

• Provides services per plan of care.
• Performs initial evaluation and regular reevaluations.
• Initiates and revises the plan of care.
• Performs preventive and rehabilitative nursing procedures.
• Prepares clinical and progress notes and summaries of care conferences.
• Coordinates services and informs changes in patient condition.
• Counsels patient and family in nursing needs.
• Processes orders and completes certifications/recertifications.
• Determines nursing needs per patient.
• Supervises and teaches other nursing personnel.
• Conducts patient care conferences.
• Participates in peer review and quality improvement.
• Provides total patient care and takes on-call duties.
• Completes OASIS actions per agency timelines.

Responsibilities

What You’ll Do:

• Provides services in accordance with the plan of care.
• Makes the initial evaluation visit and regularly reevaluates the patient’s nursing needs.
• Initiates the plan of care and necessary revisions.
• Initiates appropriate preventive and rehabilitative nursing procedures.
• Prepares clinical and progress notes for each patient visit and summaries of care conferences on his/her patients in a timely manner as per Agency policy.
• Coordinates services.
• Informs personnel of changes in the condition and needs of the patient.
• Counsels the patient and family/significant others in meeting nursing and related needs.
• Processes orders and notifies physician of patient needs and changes in condition. Completes certification/recertification orders and discharge summaries.
• Determines the amount and type of nursing needed by each individual patient.
• Supervises and teaches other nursing personnel.
• Conducts patient care conferences on patients assigned to his/her care.
• Participates in peer review and Quality Assessment and Performance Improvement as assigned.
• Gives total patient care as needed; Takes on-call duty nights, weekends and holidays, as assigned.
• Completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges and significant change in condition in accordance with Agency defined time frames.
• Appropriately utilizes ICD-10 codes.
• Performs other related duties as assigned.

Pay Range: USD $72,000.00 - USD $77,000.00 /Hr.
