Job summary

Professional Case Management in Nevada is hiring RN-Case Managers to deliver in-home healthcare case management services, improving clients’ health and quality of life. You will work with a set group of clients and can set your own schedule, enabling time for school, travel, or family activities.

Ideal candidates hold an RN license, have at least 1 year of nursing experience, and possess strong communication and interpersonal skills.

Qualifications

• RN license in state(s) of practice.
• Minimum of one (1) year of RN nursing experience.
• Previous nurse/case management experience preferred.
• Licensed driver with insurance.
• Current CPR certification and TB Test.

Responsibilities

• Create home plans of care for clients and ensure client care goals are met.

• Orders medical supplies, audits home charts and maintains accuracy for authorization paperwork.

Essential Functions of the Job

• The Case Manager‑ RN has long‑term, regular caseloads and provides supervision of ancillary staff (RNs, LPNs, CNAs, PCAs) to provide a holistic approach to care which includes educating and advocating for client’s individual needs:
