Job summary

Professional Case Management (PCM) is hiring RN-Case Managers in Texas to provide in-home case management services. You will create home care plans, serve as the primary liaison between clients and PCM, and accompany clients to physician visits as needed.

We offer flexible scheduling, full benefits after 30 days, and opportunities for professional development within a long-standing home health leader serving veterans and other clients for over 30 years.

Qualifications

• RN license in Texas or practice state
• Minimum of one year RN nursing experience
• Licensed driver with insurance
• Current CPR certification and TB Test

Responsibilities

• Creates home plans of care for clients and ensures client care goals are met
• Functions as the main point of communication between clients/families and the company
• Attends physician visits with clients as needed
• Orders medical supplies, audits home charts and maintains accuracy for authorization paperwork
