Job summary

St. Croix Hospice is seeking a Registered Nurse Case Manager to deliver patient-centered care in Monticello, MN. You will assess needs, develop individualized care plans with patients and families, administer medications, and support symptom management in home settings.

This full-time role requires RN licensure and 1+ year acute care experience; hospice/home health experience preferred. On-call rotations and reliable transportation are required, with a comprehensive benefits package provided.

Qualifications

Responsibilities

• Educate patients and caregivers on care techniques, medications, diet, and symptom management.
• Rotate on-call duties and ensure seamless care transitions.
• Must have reliable transportation, valid driver’s license, and proof of auto insurance.

As a Registered Nurse Case Manager, you serve as the cornerstone of patient care, delivering individualized, holistic support to patients in their homes. You’ll collaborate with a care team to develop a personalized plan of care, lead meaningful end‑of‑life conversations, and ensure exceptional symptom management with empathy and clinical excellence.

The starting salary range for this role is $73,000-$99,000 annual base salary.
