Job summary

CareAparent is hiring a Registered Nurse Case Manager (RNCM) to perform in-home assessments, develop and adjust care plans, and supervise home health aides. The RNCM will educate patients and families while coordinating care across therapies and providers to keep seniors safely at home.

Responsibilities include medical documentation, OASIS processes, and on-going care oversight. This role requires travel to client homes within the local area and weekend/holiday on-call rotation.

Qualifications

• Current Minnesota RN license.
• Experience with seniors and client assessments.
• Excellent customer relations and interpersonal skills.
• Excellent verbal/written communication skills.
• Valid driverlicense and insured personal vehicle.
• Willingness to travel to client homes.
• On-call availability for rotating weekends and holidays.

Responsibilities

• Conduct initial home health care OASIS assessments for Starts of Care, Resumptions of Care and Recertifications.
• Work within primary zone (local area), covering other zones as needed.
• Develop plan of care based on physician orders and initial client assessment and monitor/update as conditions change.
• Facilitate referrals to interdisciplinary teams (PT, OT, Speech Therapy, Falls Prevention).
• Perform skilled nursing tasks in coordination with Clinical Manager and scheduling.
• Oversee and supervise LPN visits and ensure plan of care is followed.
• Provide clinical oversight, delegation, and leadership to licensed and unlicensed staff.
• Prepare visit/shift reports and patient records summaries; lead care team conferences.
• Coordinate and supervise on-site CNA/PCA/HHA visits and ensure plan of care is followed.
• Report changes in client condition to physicians and Clinical Manager.
• Prepare medical documentation using CareAparent EMR platforms.

Registered Nurse Case Manager (RNCM) is responsible for conducting in-home assessments and re-evaluations, assisting in preparing care plans, performing medication set-ups, and supervising HHA's and CNAs. This position will be responsible for identifying, creating, and following the home health care plan for admission under a home health episode/authorization while ensuring to educate patients, families, caregivers, and other providers to safely perform care. Pay range: $37.50 - $46.00/hour

Duties & Responsibilities

• Conduct initial home health care OASIS assessments for comprehensive care, including Starts of Care, Resumptions of Care, and Recertifications
• Work within primary zone (local area), covering other zones as needed
• Develop a plan of care based on physician orders and initial client assessment and continue to monitor, revise, and update based on ongoing assessments or changes of condition.
• Facilitate referrals to interdisciplinary teams (Physical Therapy, Occupational Therapy, Aroma Therapy, Speech Therapy and Falls Prevention).
• Perform skilled nursing tasks as assigned in coordination with the Clinical Manager and CareAparent scheduling.
• Oversee and supervise Licensed Practical Nurse (LPN) visits and ensure plan of care is being followed.
• Provides clinical oversight, delegation, and leadership to licensed clinicians and unlicensed care team members.
• Prepare visit/shift reports, patient records summaries, and lead care team case conferences with the care team.
• Coordinate, oversee, and supervise on-site CNA / PCA / HHA visits and ensure plan of care is being followed.
• Report on changes in client condition to physicians and Clinical Manager.
• Prepare appropriate medical documentation of client assessments and visits using CareAparent EMR platforms.
