Job summary

St. Croix Hospice seeks a Registered Nurse Case Manager to deliver holistic, in-home patient care. You will collaborate with a care team to create care plans, lead end-of-life conversations, and ensure symptom management with compassion.

The role requires an RN credential and at least 1 year of acute care experience, with hospice/home health preferred. On-call duties and a focus on dignity and comfort are essential.

Qualifications

• Associates, Bachelors, or Masters of an accredited nursing program (RN).
• 1+ years of acute care experience; hospice/home health preferred.
• Ability to assess patients and families to determine hospice needs.
• Educate patients and caregivers on care techniques and medications.
• Must have reliable transportation and valid driver's license.

Responsibilities

• Complete initial and ongoing comprehensive assessments.
• Develop individualized plans of care with patient and family.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques, diet, and symptom management.
• Rotate on-call duties and ensure seamless care transitions.
