Job summary

Adaptive Home Health, LLC is expanding in Detroit and seeks a dedicated RN Case Manager to deliver in-home care using an AI-native platform that streamlines charting and documentation.

You will manage comprehensive home visits, coordinate care with physicians, and supervise field staff while maintaining high patient outcomes and strong referral relationships.

Qualifications

• Willingness to drive between patient homes and maintain reliable transportation.

Responsibilities

• Conduct comprehensive in-home assessments with skilled documentation (OASIS, SOC/recert/ROC, discharge).
• Provide direct, high-acuity patient care with focus on wound care.
• Develop and manage the plan of care with physicians; supervise LPNs and HHAs.
• Represent Adaptive to patients, families, and referral sources.

• Comp: $64.10 per point

• Mileage in addition to productivity comp

• Schedule: Full-time Monday–Friday + some weekends (as needed)

The mission of this role

Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on.

Objectives

1. Carry a full, high-quality caseload: ramp to ~30 productivity points/week at target visit quality, with OASIS accuracy that holds up to review.

2. Own your patients’ outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality.

3. Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent.

4. Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows.

What you’ll own

• Comprehensive in-home assessments and skilled documentation (OASIS, SOC/recert/ROC, discharge).

• Building and managing the plan of care with physicians; supervising LPNs and HHAs on your cases.

• Being the face of Adaptive to patients, families, and the referral sources who feed your pod.

Requirements

• Active Michigan RN license

• Reliable transportation, valid driver’s license, auto insurance

• Home health / OASIS experience strongly preferred
