In this vital role, you’ll serve as a clinical leader and patient advocate—coordinating care, guiding families, and ensuring seamless, compassionate support across homes, nursing facilities, and assisted living communities.

What You’ll Do

As an RN Case Manager, you’ll play a central role in shaping the patient experience by:

• Conductingcomprehensive patient assessmentsand developing individualized, patient-centered care plans
• Collaborating closely with aninterdisciplinary care teamto ensure seamless communication and coordination
• Managingdischarge planning and transitions of care, ensuring patients and families feel supported every step of the way
• Monitoring patient progress andadapting care plansas needs evolve
• Educating and empowering patients and caregivers with clear guidance, compassion, and advocacy
• Maintaining accurate, timely documentation in compliance with healthcare regulations
• Staying current on best practices in care management to continuously improve patient outcomes

On-Call Expectations

• Participation inbackup on-call rotation is requiredas part of this role
• Backup on-call supports patient needs after hours, weekends, or holidays as scheduled
• On-call responsibilities areshared and planned in advanceto support work-life balance

What We’re Looking For

• CurrentRegistered Nurse (RN) licensein the state of Missouri
• Strong background inpatient assessment and case management.
• Experience ingeriatrics, long-term care, home health, or hospiceis highly valued
• Knowledge of discharge planning and managed care principles preferred
• Excellent communication, organization, and time-management skills
