Job summary

Trinity Health Livingston in Michigan is seeking a Registered Nurse Case Manager to lead discharge planning and drive clinical, financial and utilization outcomes across the care continuum. This contingent, part-time role includes 0 FTE status with no benefits at the Howell/ Brighton campus. Orientation will run 6–8 weeks, 8 a.m.

to 5 p.m., Monday through Friday. You will assess patient goals, coordinate with families and the interprofessional team, review level-of-care appropriateness daily,

Qualifications

• Current Michigan Nursing License.
• Graduation from accredited nursing program: BSN is required.
• Experience: Three (3) years recent experience in direct patient care as an RN with demonstrated clinical expertise or equivalent experience and expertise as a Case Manager.
• Individuals serving as case managers for the health system shall obtain CMSA certification within three years from their date of employment in the role or be in the process of obtaining certification as a case manager.

Responsibilities

• The RN Registered Nurse Case manager is responsible for conducting a focused assessment of patient goals, as well as biophysical, psychosocial, environmental, economic/financial and discharge planning needs.
• Review, on a concurrent basis, the appropriateness, intensity, and timeliness of the level of care, diagnostic testing and clinical procedures, quality and clinical risk issues and documentation completeness.
• Develop and document a discharge plan through collaboration with the patient, family and interdisciplinary team. Ensures that all activities to facilitate and coordinate the plan are being implemented and that the plan is modified as needed based on patients’ evolving needs.
• Discuss the plan of care daily with physicians and care team members to ensure the continued need for acute care hospitalization, while exploring the most desirable/appropriate post-acute care setting to meet the patients' needs and goals.
