Job summary

Detroit Medical Center is seeking a PRN RN Case Manager to coordinate care across the continuum at DMC Receiving Hospital in Detroit, MI. The role emphasizes appropriate level of care, timely transitions, and alignment with patient preferences, while balancing clinical and regulatory requirements.

Qualifications

  • Graduate from an accredited school of nursing. Bachelor’s degree in Nursing or other health‑related field, or equivalent education and experience.
  • Two years of acute hospital patient care experience. Acute hospital case management experience preferred.
  • License to practice as a Registered Nurse in the State of Michigan.
  • Accredited Case Manager (ACM) preferred.
  • Must complete Tenets InterQual education course within 30 days of hire (and at least annually thereafter) and achieve a score of 85 or better. Must complete and demonstrate competency in using the Tenet Case Management documentation system within 30 days of hire. Attendance at hospital and department orientation is required. Department orientation includes review of Tenet Case Management and Compliance policies, InterQual, Transition Management, Utilization Management, and other case‑management topics.

Responsibilities

  • Accurate medical necessity screening and submission for Physician Advisor review.
  • Care coordination across teams and departments.
  • Transition planning assessment and reassessment.
  • Implementation or oversight of transition plans.
  • Leading and facilitating multidisciplinary patient care conferences.
  • Managing concurrent disputes.
  • Making appropriate referrals to other departments.
  • Identifying and referring complex patients to Social Work Services.
  • Communicating with patients and families about the plan of care.
  • Collaborating with physicians, office staff, and ancillary departments.
  • Leading and facilitating Complex Case Review meetings.
  • Assuring patient education is completed to support post‑acute needs.
  • Timely completion of concise documentation in the Case Management system.
  • Maintenance of accurate patient demographic and insurance information.
  • Identification and documentation of potentially avoidable days.
  • Identification and reporting of over‑ and under‑utilization.
  • Other duties as assigned

Job Description

The RN Case Manager is responsible for coordinating care across the continuum to help patients achieve optimal health, access to care, and appropriate utilization of resources. The role ensures that care is provided at the appropriate level of care based on medical necessity and that transitions are timely, safe, and aligned with patient preferences. Facility Description

DMC Detroit Receiving Hospital is Michigan’s first Level I Trauma Center and one of the busiest emergency departments in the region. It also hosts the state‑wide leading 24/7 hyperbaric oxygen program, Metro Detroit’s first certified primary stroke center, and the nationally recognized DMC Rosa Parks Geriatric Center of Excellence. The facility was the first and largest verified burn center in the state and is one of only 43 in the nation.
