Angels of Care Home Health is hiring a Registered Nurse Case Manager to lead a team of clinicians delivering patient care. You will manage a caseload, coordinate with an interdisciplinary team, and train new staff.
The role includes field days and office days with regular local travel to supervise and support care delivery. Responsibilities include developing treatment plans, conducting assessments, and ensuring documentation aligns with agency standards.

As a Registered Nurse (RN) - Case Management, you will lead a team of dedicated clinicians in delivering exceptional client care. You will manage a case load of clients, partner with an interdisciplinary team, train and onboard new hires, and manage client referrals. This is a great opportunity for a collaborative, solution-focused RN to make a meaningful impact.
This role includes field days (for staff supervision and oversight of patient care) and office days. Administrative tasks on field days can generally be completed off-site unless office presence is needed for operational reasons.
