The role you'll contribute: The RN Care Manager collaborates with patients, families, social workers, nurses, physicians, and the interdisciplinary team to ensure patient-centered care coordination and progression through the continuum of care. The RN Care Manager promotes efficient, cost-effective care through resource monitoring and escalates clinical care issues as needed. Reporting to the Care Management Supervisor, Manager, or Director of Nursing, they evaluate post-hospital needs, develop transition of care plans, and initiate their implementation before patient discharge.
The RN Care Manager aims to optimize patient flow, ensure safe transitions, enhance patient satisfaction and safety, prevent readmissions, and manage length of stay. They participate in daily multidisciplinary rounds, focusing on care coordination, discharge planning, and understanding medical necessity. They facilitate collaborative management across the care continuum, remove barriers to timely care, and educate team members on resource utilization, medical necessity, CMS CoP for Discharge Planning, and care coordination. They are knowledgeable about post-hospital services such as Home Health, Infusion Services, Durable Medical Equipment, Palliative Care, Hospice, Outpatient Services, and community programs. They adhere to departmental policies, ensure quality and compliance, and uphold outstanding customer service.
