Job summary

AdventHealth seeks a dedicated Registered Nurse to join our home health team in Sebring, FL. You will evaluate safety and infection control in patients’ homes, review histories and lab data, and implement and supervise care plans through the health care team.

Excellent communication with physicians and families is essential. The role requires an RN license (FL or eligible), a driver’s license, and BLS certification.

Qualifications

• RN licensed in Florida or eligible for licensure in Florida.
• Strong clinical assessment and care planning abilities.
• Proficient with electronic medical records and basic computer skills.

Responsibilities

• Assess home environment for safety and infection control.
• Review patient history and test results; report changes to physician.
• Coordinate care plans; supervise home health team activities.
• Provide skilled nursing care and prescribed treatments in home settings.
• Engage patients and families using health coaching and motivational interviewing.
• Maintain up-to-date clinical documentation and care plans.

Job Description:

• Evaluates the home environment for safety, infection control, and community resource needs.
• Reviews patient history, physical diagnostics, and laboratory data, and reports abnormal results to the physician.
• Implements care plans through direct patient care, coordination, delegation, and supervision of healthcare team activities.
• Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments in various home situations.
• Uses motivational interviewing and health coaching techniques to engage stakeholders in care management.
• Informs the physician, clinical manager, and healthcare team of changes in the patient’s condition and needs.
• Maintains updated clinical records, meeting documentation deadlines for certification, re-certification, and care plan updates.
• Provides comprehensive assessment, planning, implementation, and evaluation for a caseload of home patients.
• Optimizes daily schedules to support productivity, efficiency, and best practice visit utilization.
• Other duties as assigned.
• Assesses physical, functional, psychosocial, cultural, cognitive status, and discharge planning needs of home care patients.
• Formulates patient‑specific care plans with the patient, family, and physician, establishing individualized, realistic, measurable goals.
• Mileage reimbursement provided: $0.76 per mile

Knowledge, Skills, and Abilities:

• Functions with a high degree of independence [Required]

• Home Care Regulations and Third-Party Reimbursement as it impacts care delivery [Preferred]
• Current IV Therapy skills [Preferred]

Education:

• Associate's of Nursing [Required]
• Bachelor's of Nursing [Preferred]

Work Experience:

• 1+ relevant clinical nursing experience [Required]
• Recent, relevant experience in a Medicare-certified home health agency as a case-manager [Preferred]

Licenses and Certifications:

• Registered Nurse (RN) [Required]
• Driver's License (DL) [Required]
• Basic Life Support - CPR Cert (BLS) [Required]
• Certified for Oasis Specialist-Clinical (COSC) [Preferred]

Pay Range:

$33.11 - $61.60
