$90,000–$105,000 + $5,000 Sign-On Bonus | Hybrid Role | Greater Boston Area

Join our mission-driven team as an RN Care Manager—a hybrid position blending impactful in-home visits with the convenience of remote coaching. We offer competitive pay, a generous $5,000 sign-on bonus, and no night shifts, weekends, or on-call requirements!

Key Responsibilities:
  • Conduct in-home visits across assigned territories to assess safety, monitor health status, and provide medication reviews and clinical coaching.

  • Guide and support caregivers via our intuitive digital platform, offering education, referrals, and care plan follow-ups.

  • Build trust-based relationships with caregivers, fostering long-term positive health outcomes and preventing burnout.

  • Accurately document all interactions and care plans, maintaining full compliance with healthcare regulations and standards.

  • Participate in team projects and contribute to ongoing program improvements.

Required Qualifications:
  • Active Registered Nurse (RN) license in the state of Massachusetts.

  • Minimum 2 years of direct care experience, ideally with elders, adults with disabilities, or patients with complex medical needs.

  • Strong background in care planning, assessment, and case management.

  • Comfortable using web-based platforms and tools like MS Office and EMR systems.

  • Reliable transportation, valid driver’s license, and current auto insurance.

  • Bilingual candidates are encouraged to apply—language skills may increase compensation.
