This full-time, day-shift role emphasizes interdisciplinary collaboration and comprehensive care coordination. Ideal candidates hold an Associate degree in nursing with CT RN licensure and BLS, with 2 years in Medical-Surgical or Critical Care preferred.

Qualifications

What You'll Do

• Implement and monitor patient care plans.
• Monitor, record, and communicate patient condition as appropriate.
• Serve as a primary coordinator of all disciplines for well-coordinated patient care.
• Note and carry out physician and nursing orders.
• Assess and coordinate patient's discharge planning needs with the healthcare team.
• Demonstrate knowledge and skills necessary to assess patients' needs.
• Delegate aspects of care to other nursing personnel.
• Participate in daily interdisciplinary patient rounds.
• Develop, implement, and revise individualized plans of care.
• Participate in activities for professional growth and development outside of patient care.

Requirements

• Education: Associate degree required; BSN preferred.
• Experience: Two years of Medical-Surgical/Critical Care experience preferred.
• Licenses & Certifications: Current Connecticut RN license. BLS certification by the American Heart Association.
