Meadow Park Rehabilitation & Healthcare Center seeks a skilled Registered Nurse (RN) to manage care on a modern subacute unit. As charge nurse, you will coordinate patient care using the nursing process, collaborate with the medical team, and support residents and families.

Applicants must hold NY RN licensure, have LTC experience, know EMR systems, and maintain BCLS/ACLS/PALS. Strong communication is essential for this role.

Qualifications

• Licensed as an RN in New York State.
• Experience in a Long Term Care facility preferred.
• Proficient with EMR systems.
• BCLS, ACLS or PALS required.
• Excellent communication skills.

Responsabilités

• Manage individualized resident care using the nursing process.
• Utilize clinical resources and justify patient care decisions.
• Collaborate with physicians and interdisciplinary team to address resident needs.
• Provide physical and psychological support to patients and families.
• Monitor residents’ conditions and administer medications and treatments.
• Supervise assigned nursing staff and ensure protocol adherence.
• Complete intake/outtake records and frequent observations per protocols.
• Perform related duties as required.

Meadow Park Rehabilitation & Healthcare Center is actively seeking an RN to manage our modern, newly renovated subacute unit.

As a charge nurse you will be responsible for:

• Managing the individualized resident care by promoting and monitoring residents' health through the nursing process.
• Utilizes clinical resources in implementing patient care and provides rationale for patient care decisions.
• Collaborating with physicians and interdisciplinary team members to resolve all residents' problems and needs
• Providing physical and psychological support to patients and their family members
• Monitoring residents' conditions and administering medication & treatments
• Supervising assigned team members.
• Completes Frequent Observations and the Intake/Outtake Record according to established protocols.
• Performs related duties, as required.

Requirements

• Licensed as a Registered Nurse in the State of NY
• Past experience working in a Long Term Care Nursing Facility
• Knowledge of EMR systems
• BCLS, ACLS or PALS, required.
• Excellent communication skills
