Job summary

Meadow Park Rehabilitation & Healthcare Center is seeking a Registered Nurse (RN) to manage care on a modern, renovated subacute unit in New York. The role emphasizes nursing process, collaboration with physicians, and supporting residents and families.

Requirements include NY RN licensure, LTC experience, EMR knowledge, BCLS/ACLS/PALS, and excellent communication skills. Sign-on bonus available and competitive on-site position.

Responsibilities

Meadow Park Rehabilitation & Healthcare Center is actively seeking an RN to manage our modern, newly renovated subacute unit.

As a charge nurse you will be responsible for:

• Managing the individualized resident care by promoting and monitoring residents' health through the nursing process.
• Utilizes clinical resources in implementing patient care and provides rationale for patient care decisions.
• Collaborating with physicians and interdisciplinary team members to resolve all residents' problems and needs
• Providing physical and psychological support to patients and their family members
• Monitoring residents' conditions and administering medication & treatments
• Supervising assigned team members.
• Completes Frequent Observations and the Intake/Outtake Record according to established protocols.
• Performs related duties, as required.

Requirements:

• Licensed as a Registered Nurse in the State of NY
• Past experience working in a Long Term Care Nursing Facility
• Knowledge of EMR systems
• BCLS, ACLS or PALS, required.
• Excellent communication skills
