Schedule

Monday-Friday with home-based care focus.

On-call every third or fourth weekend depending on staffing levels.

Key Responsibilities

• Deliver direct and indirect nursing care according to Care Plans, managing complex clinical situations independently.
• Assess, prioritize, and document patient needs; interpret diagnostic results and initiate appropriate interventions.
• Collaborate with multidisciplinary teams to develop and update care plans; involve patients and families in care decisions.
• Coordinate and document 24-hour care; communicate patient changes and ensure seamless care transitions.
• Perform home visits and complete all required electronic medical record documentation.
• Support Quality Improvement initiatives and other duties as assigned.
