About the Role

Join STAT Home Health in Conroe, TX to provide excellent patient care within a supportive team environment. We are a rapidly expanding Home Health Care and Hospice agency spanning six states, employing over 2,400 healthcare professionals.

Essential Duties

• Supervise patient care
• Coordinate all aspects of patient care; initiate and update the plan of care
• Complete all documentation timely, including Oasis, 485, and order submission
• Supervise, orient, and teach other clinical personnel as needed
• Ensure compliance with local, state, and federal laws, including problem resolution and complaint investigation

Qualifications

• Current Texas Registered Nurse (RN) license
• Minimum 1 year of clinical experience as an RN or LPN
• Current CPR Certification
• Compassionate with excellent time management skills
• Reliable transportation for patient visits
