Job summary

Southeastern Renal Dialysis is seeking an experienced RN to lead and coordinate our Home Dialysis Program for End Stage Kidney Disease patients. This Clinical Team Lead will supervise RNs, LPNs, and support staff, ensuring high-quality care, compliance, and patient outcomes in a dynamic clinical setting.

Responsibilities include training patients and partners, overseeing care plans, and acting as a clinical resource while supporting on-call coverage.

Qualifications

• Current RN license in applicable state, maintained in good standing.
• 12 months of clinical nursing experience.
• 6 months of experience in the home dialysis modality.
• CPR certification required prior to patient care assignment.
• Ability to distinguish primary colors.
• Must complete Home Therapy Nurse Orientation and maintain ongoing competency.
• Strong English communication skills, both written and verbal.
• Basic computer skills; proficiency in clinical applications within 90 days of hire.

Description

Southeastern Renal Dialysis is seeking an experienced Registered Nurse (RN) – Clinical Team Lead to oversee and coordinate our Home Dialysis Program for patients with End Stage Kidney Disease (ESKD). This leadership role is responsible for delivering high‑quality patient care while supervising and supporting RNs, LPNs, Patient Care Technicians, and Home Therapy Care Team Assistants. The Clinical Team Lead ensures clinical excellence, regulatory compliance, staff development, and strong patient outcomes.

Essential Duties And Responsibilities

• Assess, plan, implement, and evaluate care for assigned home dialysis patients, providing ongoing clinical oversight.
• Train and educate patients and care partners on safe, effective home dialysis therapy and treatment modalities, including transplant options.
• Monitor treatment data, laboratory results, and patient response; adjust care plans and communicate updates to physicians and the interdisciplinary team.
• Administer medications as prescribed and respond appropriately to clinical changes or emergencies.
• Ensure accurate lab collection, processing, and documentation in compliance with regulatory standards.
• Lead and oversee the home dialysis training program, ensuring high standards of patient competency and safety.
• Supervise, mentor, and support clinical staff; assist with orientation and ongoing training.
• Serve as a clinical resource and act as Nurse in Charge when assigned.
• Ensure compliance with state, federal, and CMS regulations.
• Support and drive quality improvement and infection control initiatives.
• Participate in scheduling and rotating on‑call coverage, including nights, weekends, and holidays as required.

Requirements

• Current RN license in applicable state. License must be maintained as current and in good standing.
• 12 months of clinical experience in providing nursing care and 6 months of experience in the specific modality for which the nurse will provide home dialysis training.
• Previous experience in providing nursing care to patients on maintenance dialysis preferred.
• CPR certification required prior to patient care assignment.
• Confirmation of ability to distinguish all primary colors.
• Must successfully complete the Home Therapy Nurse Orientation program and maintain annual demonstration of skills and competency applicable for the modality assigned.
• Demonstrated working knowledge of the English language and ability to communicate verbally and in writing.
• Must have basic computer skills; proficiency in all clinical applications required within 90 days of hire.
• Must meet any practice requirement(s) for the applicable state.
• Ability to meet minimum hiring standards which may include additional background clearances and orientation requirements if applicable to the program assigned to for employment.
