Job summary

NHC HomeCare Midlands in West Columbia, SC is seeking a compassionate Registered Nurse to provide in-home care to patients. You will assess, plan, implement, and evaluate nursing care using the nursing process, and collaborate with the interdisciplinary team to ensure quality outcomes.

The ideal candidate holds a current RN license in SC or a compact state, has at least one year of RN experience (home care experience preferred), and can educate patients and caregivers on health management.

Qualifications

• Unencumbered RN license in SC or compact state.
• Minimum one year RN experience.
• Home care experience desirable.

Responsibilities

• Apply nursing process to identify patient goals, plan, implement, and evaluate care.
• Assess physical, psychosocial, and environmental factors to develop a comprehensive nursing care plan.
• Collaborate with the interdisciplinary team to ensure personal, medical, and rehabilitation care.
• Educate patient and caregiver in health care and disease management and document ongoing needs.

Licensure:

• Unencumbered current Registered Nurse license in the state where the HomeCare agency is located or compact state, if applicable.
• Minimum one (1) year experience as a RN / professional nurse.
• Experience in home care is desirable.

Position Highlights:

• Utilizes the nursing process to identify and achieve patient goals: assessment, planning, implementation, and evaluation.
• Assesses the physical, psychosocial, and environmental factors that affect a patient’s health to develop a comprehensive nursing care plan which will attain the patients desired health outcomes in a culturally comfortable way.
• Collaborates with the interdisciplinary team to assure personal care, medical care and rehabilitation provide for optimal patient health and well-being.
• Teaches patient/caregiver in various aspects of health care and disease management appropriate to needs/level of education and understanding and documents continuing needs and levels of patients/caregivers understanding.
