Job summary

Lakeland Nursing and Rehab in Wanaque, New Jersey, is seeking compassionate Registered Nurses to join our dedicated care team across all shifts. You will provide direct patient care, administer medications, and collaborate with the healthcare team to deliver comprehensive resident care.

The ideal candidate holds an active New Jersey RN license, 1–2 years SNF experience, and strong communication, organizational, and critical thinking skills.

Qualifications

• Active RN license in the State of New Jersey.
• Minimum 1-2 years of SNF (Skilled Nursing Facility) experience.
• Excellent communication, organization, and critical thinking abilities.

Responsibilities

• Provide direct patient care to residents and assess, plan, implement, and evaluate care plans.
• Administer medications, perform wound care, and monitor vital signs.
• Collaborate with the healthcare team to ensure comprehensive resident care.
• Develop and implement educational programs for residents and families.
• Communicate effectively with residents, families, and team members.

Key Responsibilities

• Provide direct patient care to residents, including assessing, planning, implementing, and evaluating care plans

• Collaborate with other members of the healthcare team to ensure comprehensive care

• Communicate effectively with residents, families, and healthcare team members
• Maintain accurate and detailed records of patient care and treatments
• Participate in quality assurance and improvement activities to ensure high-quality care

• Contribute to quality assurance initiatives, budget planning, and internal committees
• Ensure accurate MDS documentation and coordinate discharge planning with families
• Provide individualized, compassionate care to residents and their loved ones
