The Registered Nurse (RN) in Home Health provides and directs nursing care to patients in their homes as prescribed by physicians, complying with laws, regulations, and agency policies.

• Coordinates the total care plan with healthcare professionals and ensures continuity of care by planning and exchanging information with physicians, agency staff, patients, families, and community resources within a Point of Care setting.
• Performs initial and/or comprehensive nursing evaluations, determines the primary focus of care, develops the plan of care within state guidelines, and submits accurate documentation in collaboration with physicians.
• Conducts patient assessments and develops appropriate care plans at designated times.
• Ensures patients meet home health eligibility and medical necessity guidelines as defined by payers.

License Requirements

• Current RN licensure in the state of practice
• Current CPR certification
• Valid driver’s license, vehicle insurance, and access to reliable transportation
