Job summary

Sevita is seeking a Registered Nurse to join our community health team in Illinois. You will conduct assessments, implement care plans, and monitor patients' conditions to deliver high-quality care.

We value critical thinking, teamwork, and compassionate patient advocacy. You will collaborate with physicians and staff to enhance outcomes and provide staff training for safe, effective care.

Qualifications

• Accredited RN degree required or equivalent.
• Current state RN license in Illinois.
• CPR/First Aid certification must be current.

Responsibilities

• Conduct comprehensive patient assessments and implement care plans.
• Monitor patient conditions and adjust care as needed.
• Provide clinical oversight and training to staff to ensure quality care.
• Collaborate with physicians and external partners to prioritize patient outcomes.

• Education: Accredited RN degree, current state RN license, and current CPR/First Aid certification
