Job summary

CHOSEN VILLAGE HOME HEALTHCARE is seeking a Registered Nurse to provide direct patient care across home health settings. You will administer medications, monitor patient conditions, and coordinate with the healthcare team to implement care plans.

The role requires an active RN license in the state and strong communication skills to educate patients and families. A BSN is preferred and the position involves start of care and OASIS assessments in an in-person setting.

Qualifications

• Current RN license in the state of practice.
• BSN degree preferred.
• Minimum nursing experience in a clinical setting.
• Proficient in occupational health procedures including catheterization, aseptic technique, injections, and related assessments.
• Strong knowledge of nursing principles, practices, and standards of care.
• Excellent communication and interpersonal skills.
• Ability to work effectively as part of a multidisciplinary healthcare team.
• Attention to detail and strong organizational skills.
