Laurellwood Post-Acute and Rehab is seeking a compassionate and skilled Registered Nurse (RN) to join our dedicated team of healthcare professionals. As a vital member of our interdisciplinary care team, you will provide high-quality, patient-centered care to our residents, while collaborating with other healthcare professionals to ensure seamless transitions and optimal outcomes.

Responsibilities:

• Deliver exceptional nursing care to our residents, including assessing, planning, implementing, and evaluating individualized care plans
• Collaborate with the interdisciplinary care team to develop and implement comprehensive care plans that meet the unique needs of each resident
• Conduct thorough assessments, identify changes in condition, and report findings to the charge nurse and medical director
• Administer medications, treatments, and therapies as ordered by the medical director
• Monitor and report adverse reactions to medications and treatments
• Maintain accurate and timely documentation of resident care, including assessments, treatment plans, and progress notes
• Participate in care conferences, discharge planning, and family education to ensure continuity of care
