Portsmouth Health and Rehab in Portsmouth, OH is hiring an experienced RN to supervise nursing care and ensure high-quality resident services. You will lead LPNs, CNAs, and other staff, and participate in developing and implementing care plans in alignment with regulatory standards.

The role requires Ohio RN licensure, a background in geriatric care, and a commitment to compassionate, patient-centered care.

Qualifications

• High School Diploma or equivalent.
• RN in OH.
• Previous LTC/SNF experience preferred.
• Serious applicants.
• Interest in the geriatric population.

Responsibilities

• Supervises nursing care provided by LPNs, CNAs, and others.
• Reviews and implements nursing procedures and systems.
• Assesses and evaluates systems facilitating quality care.
• Uses a systematic nursing process for individualized care.
• Contributes to resident assessments (MDS/CAA) and plan of care.
• Evaluates residents’ responses to interventions.
• Assists DON and ADON to plan and direct facility operations per standards.

  • Supervises nursing care provided by Licensed Practical Nurses (LPN), Certified Nursing Assistants (CNA), and others.
