Job summary

ACG Hospice is seeking a Registered Nurse to provide compassionate hospice care, assessing patients and developing care plans while coordinating with families and the care team. You will lead the care team to ensure quality, patient-centered outcomes.

We offer development opportunities, supportive leadership, and paths for advancement within Agape Care Group. Nursing license and hospice experience are highly valued.

Qualifications

• Heart to serve patients and families with empathy.
• Graduate of an accredited nursing school and current RN license.
• 2+ years of nursing experience in a clinical setting; hospice experience preferred.
• Must have reliable transportation and physical ability to lift substantial weight.

Responsibilities

• Evaluate patients and create individualized care plans.
• Communicate with patient, family and care team to ensure coordinated care.
• Drive the care team to deliver high-quality, compassionate hospice care.

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care
• Education: Graduate of an accredited school of nursing with a current state license as a registered nurse
• Experience: 2+ years of nursing experience in a clinical care setting (hospice experience preferred)
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 80-100 lbs and bear the weight of an average adult effectively.
