Agape Care Group is seeking a Registered Nurse to evaluate patients, design care plans, and coordinate with families and the care team. You will lead the team to ensure every patient receives high-quality hospice care.

We offer a supportive leadership team, professional development opportunities, and room for advancement within our hospice network across multiple states.

Qualifications

• Heart to serve patients and families and a passion for providing the best possible care.
• Graduate of an accredited school of nursing with a current state license as a registered nurse.
• 2+ years of nursing experience in a clinical care setting; hospice experience preferred.
• Reliable transportation and ability to lift up to 100 lbs as required.

Responsibilities

• Evaluate patients and create care plans in collaboration with the care team.
• Communicate with patients, families, and team members to ensure quality care.
• Serve as driver of the care team to deliver compassionate hospice care.
• Participate in ongoing professional development and retain a positive team culture.

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care
• Education: Graduate of an accredited school of nursing with a current state license as a registered nurse
• Experience: 2+ years of nursing experience in a clinical care setting (hospice experience preferred)
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 80-100 lbs and bear the weight of an average adult effectively.
