Qualifications

• RN license required or eligible for licensure in Pennsylvania.
• Experience providing direct nursing care to residents in a long-term care setting preferred.

Responsibilities

• Provide direct patient care to residents, including administering medications, wound care, and treatments.
• Assess resident needs and develop individualized care plans to address physical, emotional, and social needs.
• Collaborate with interdisciplinary team to develop and implement comprehensive care plans.
• Monitor resident progress and adjust care plans as needed.
• Communicate effectively with residents, families, and healthcare professionals to ensure seamless care coordination.
• Participate in quality improvement initiatives to improve patient outcomes and satisfaction.
• Maintain accurate and detailed records of patient care and treatment.

About the Role:
