Job summary

St.-Joseph is seeking a Registered Nurse (RN) for Home Health in Odessa, TX. The RN will provide one-on-one, in-home care, conduct initial and ongoing assessments, manage medications, and educate patients and families on home health strategies.

The role requires an active RN license, CPR certification, and a patient-first approach. Flexible schedule and local travel are expected to support patient needs in the community.

Qualifications

• Graduate of an accredited Diploma, Associate, or Bachelor’s School of Nursing, or vocational nursing program.
• Current state license as a Registered Nurse (RN).
• Proof of current CPR certification.

Responsibilities

• Visit patients in their homes for personalized, one-on-one care visits.
• Complete initial and ongoing assessments to identify patients' needs.
• Assess patients’ conditions, chart observations, and perform vital sign checks and medication reviews.
• Administer medications as prescribed by the physician.
• Educate patients and families on home health care practices and devices that may improve quality of life.
• Communicate effectively with patients, families, and healthcare professionals.

Responsibilities

As a Home Health RN, you will:

• Visit patients in their homes for personalized, one-on-one care visits
• Complete initial and ongoing assessments to identify the physical, psychosocial, and environmental needs of home health patients
• Assess patients’ conditions, chart observations, and perform evaluation tasks including vital signs and medication review
• Administer medications as prescribed by the patient’s physician
• Educate patients and their families on proper home health care strategies and procedures, and make recommendations for devices or tools that might improve the patient’s quality of life
• Provide effective communication to patients, family members, team members, and other healthcare professionals

Qualifications
