Job summary

Agape Care Group in North Carolina is seeking a compassionate Registered Nurse to evaluate patients, develop care plans, and coordinate with families and the care team to deliver exceptional hospice care.

As a key driver of the care team, you will coordinate communications, advocate for patients, and contribute to professional development within a supportive leadership environment with opportunities for advancement.

Qualifications

• RN license required, active in state nursing board
• 2+ years nursing experience in a clinical care setting (hospice experience preferred)
• Reliable transportation and ability to lift 80-100 lbs as needed

Responsibilities

• Evaluate patients and create individualized care plans.
• Communicate with patients, families, and the care team to coordinate care.
• Serve as the driver of the care team to ensure high-quality patient care.

Essential Functions

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care
• Education: Graduate of an accredited school of nursing with a current state license as a registered nurse
• Experience: 2+ years of nursing experience in a clinical care setting (hospice experience preferred)
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 80-100 lbs and bear the weight of an average adult effectively.
