Job summary

Elara Caring seeks a Registered Nurse Home Health to contribute to our mission of providing the Right Care, at the Right Time, in the Right Place. You will assess patients, complete ongoing assessments, and administer nursing care per physician orders across home settings.

The role requires an active RN license in Indiana, 1+ year nursing experience, and the ability to travel between patient homes. Home health experience preferred but not required; driving is required with possible mileage

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.

Responsibilities

• Assessing patients to determine their care needs.
• Completing initial and ongoing patient assessments.
• Protecting patient and staff privacy per HIPAA.
• Providing nursing care according to plan of care and physician orders.
• Monitoring patient conditions and reporting changes to the care team.
• Educating patients and families on care and safety.
• Taking appropriate action when patient conditions change.
• Help ensure quality, safe, and continuous patient care.

As a Registered Nurse Home Health , you'll contribute to our success in the following ways:

What is Required?

• Current State license as a Registered Nurse.

• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.

• 1+ year of experience as a nurse in a clinical care setting.

• Home Health experience is preferred, but not required.

• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
