Accurately assesses residents' health status, educational, and environmental needs on admission and ongoing. Communicates significant changes to the interdisciplinary team (IDT). Documents assessments and develops care plans based on residents' needs, involving physicians, families, and other IDT members. Reviews and revises care plans as needed, and communicates these plans to residents, families, and staff. Implements nursing interventions such as medication administration, treatments, and IV fluids safely and promptly, with accurate documentation. Uses critical thinking to identify changes in residents' conditions, notifying physicians using SBARR. Maintains professionalism during crises, monitors care effectiveness, and uses QAPI processes for problem resolution. Keeps informed about residents' status to detect behavioral or health changes. Assists with ADLs and responds to call lights. Completes at least twelve hours of nursing-related educational offerings regularly, staying current with best practices and Bethesda policies. Performs other duties as assigned.
