Summary

Accurately assesses residents' health status, educational, and environmental needs on admission and ongoing. Communicates significant changes to the interdisciplinary team (IDT). Documents assessments and develops care plans collaboratively with residents, families, and IDT members. Reviews and revises care plans as needed. Implements nursing interventions such as medication administration, treatments, and IV fluids safely and timely. Documents interventions accurately. Uses critical thinking to manage residents' changes in condition, notifying physicians as needed. Maintains professionalism during crises. Monitors care effectiveness, addresses problems via QAPI, and stays informed about residents' status. Assists with resident care activities and completes ongoing nursing education to stay current with best practices. Performs other duties as assigned.

Job Qualifications
  • Graduation from an accredited nursing program leading to RN licensure
  • Current RN license in the state of employment
  • CPR/AED certification or willing to obtain within six months
  • Preferred: Long-term care experience
  • Critical thinking skills required
  • Excellent verbal communication skills
  • Strong time management and organizational skills
  • Ability to stand, walk, lift, and transfer patients as needed, with proper techniques and assistance
  • Manual dexterity and visual acuity required for documentation and patient care
Pay range: $29.39 - $48.20 per hour
