Responsibilities

• Conduct thorough assessments to provide personalized, comprehensive home care.
• Educate patients and families on health maintenance and disease prevention.
• Assist with Activities of Daily Living (ADLs) as needed with dignity and respect.
• Coordinate your schedule with patients according to their plan of care and preferred hours.
• Monitor and promptly report changes in patient symptoms or behavior to the care team.
• Administer medications and treatments as prescribed by the physician.
• Communicate with Case Managers and collaborating physicians to ensure unified care.
• Maintain accurate patient documentation in the EMR system.
• Provide emotional support to patients and family members as a trusted caregiver.

Pay Range
