Job Description
We are searching for a nurse who can assist our nursing team as needed with the following tasks:
Keep skilled client records updated
Can develop and implement a plan of care that meets each patient's specific needs according to supervising MD orders and with agency policies and procedures.
- Prepare POC and fax to doctor for signature
Counsels the patient and/or family in meeting nursing and related needs.
Continually communicates patient care to office staff, involved clinicians, and supervising MDs to ensure that every single patient receives the care that they deserve and need.
Completing intake/assessment of new clients
- Ensure State standards are met and being followed
- Continue Education for caregivers
- Provide necessary training's as needed for caregivers, CNA's, and Nurses
- Orientation for new employees
- Emergency or urgent client visit during business hours on Mondays and Tuesdays
- Work Holidays if they fall on Mondays and Tuesdays
- Scheduling on-call for the month using the on-call RNs
- Auditing client files for skilled and non-skilled clients
- Other assignments as needed
Qualifications
Registered Nurse Professional License, RN (required)
Valid CPR card (required)
Valid Driver’s license, car insurance, and reliable transportation (required)
Clean background check (required)
Previous experience in home care (required)
