Job summary

Preceptor Home Health & Hospice is seeking an RN Case Manager to coordinate patient care in Kenosha County. You will develop plans of care, perform assessments, deliver skilled nursing care, and educate families in the home setting.

Ideal candidates hold a Wisconsin RN license, have strong clinical and communication skills, and can manage a caseload independently. Home health experience is preferred, but not required. Competitive wages and support for professional growth are offered.

Qualifications

• Current Registered Nurse (RN) license in the State of Wisconsin.
• Graduate of an accredited nursing program.
• Home health or public health experience preferred but not required.

Responsibilities

• Develop, implement, and update individualized care plans.
• Perform comprehensive patient assessments and ongoing evaluations.
• Provide skilled nursing care and carry out physician-ordered treatments.
• Educate and counsel patients, families, and caregivers.
• Coordinate care with physicians and other healthcare professionals.
• Manage documentation, with most work in the patient's home.
• Lead and participate in case conferences and care planning discussions.
• Identify and implement discharge planning needs.
• Communicate patient status updates to physicians and care team members.
• Supervise and evaluate care by LPNs and Home Health Aides.
• Ensure equipment and services are arranged for patients.

Registered Nurse (RN) Hospice Case Manager

Location: Kenosha/Racine Counties

Wage: $38-$44 per hour

As an RN Case Manager, you will play a vital role in coordinating care, supporting patients and families, and helping individuals thrive in the comfort of their homes.

What You’ll Do

• Developing, implementing, and updating individualized plans of care
• Performing comprehensive patient assessments and ongoing evaluations
• Providing skilled nursing care and carrying out physician-ordered treatments
• Educating and counseling patients, families, and caregivers
• Coordinating care with physicians and other healthcare professionals
• Managing documentation, with a majority completed in the patient's home
• Leading and participating in case conferences and care planning discussions
• Identifying and implementing discharge planning needs
• Communicating patient status updates to physicians and care team membersSupervising and evaluating care provided by LPNs and Home Health Aides
• Ensuring necessary equipment and services are arranged for patients

What We’re Looking For

• Current Registered Nurse (RN) license in the State of Wisconsin
• Graduate of an accredited nursing program
• Home health or public health experience preferred (not required)
• Strong clinical assessment and critical thinking skills
• Excellent communication and organizational abilities
• Ability to work independently and manage a caseload effectively
• Valid driver’s license with a reliable, insured vehicle
• Physical ability to perform patient care tasks (lifting, standing, mobility support, etc.)
