Job summary

Signature HealthCARE is seeking registered nurses for skilled nursing and assisted living facilities. You will deliver direct care, supervise nursing staff, manage medications, and collaborate with a multidisciplinary team to support resident well-being.

Ideal candidates bring RN licensure, 1+ year of clinical nursing experience (long-term care preferred), and familiarity with EHR systems. Weekend shift diff available, with comprehensive benefits and growth opportunities.

Qualifications

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living (ADLs) while ensuring comfort, dignity and safety.
• Supervise nursing assistants and medication aides during shifts per policy.
• Monitor and observe residents; notify physicians or responsible parties of changes in condition.
• Develop and review individualized care plans with the interdisciplinary team.
• Complete accurate clinical documentation per facility policy and procedures.
• Coordinate admissions, transfers and discharges per established procedures.

How you Will make a difference

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.

What you Need to make a Difference

• Current, active RN license in the state of practice.

• Strong clinical observation, medication administration, and documentation skills.

• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
