Job summary

Signature HealthCARE's Princeton Transitional Care & Assisted Living invites experienced RNs to join a dedicated nursing team in the Tri-Cities area. You will provide skilled nursing care, administer medications, assist with ADLs, and support a safe, compassionate environment for residents.

We value strong clinical judgment, regulatory knowledge, and teamwork. A current RN license in TN, CPR/BLS, and LTC experience are preferred. Sign-on bonus of $7,500 is available plus robust benefits.

Qualifications

  • Current, active RN license in the state of practice.
  • CPR/BLS certification preferred (American Heart Association or American Red Cross).
  • Minimum one (1) year of clinical nursing experience; long-term care experience preferred.
  • Experience in a supervisory or charge nurse role preferred.
  • Strong clinical observation and documentation skills.
  • Knowledge of Federal and State long-term care regulations.
  • Effective communication skills both verbal and written.
  • Ability to work independently and exercise sound clinical judgment.
  • Familiarity with EHR systems.

Responsibilities

  • Provide direct nursing care to residents, including medication administration and wound care.
  • Assist residents with activities of daily living (ADLs) while maintaining dignity and safety.
  • Supervise nursing assistants and medication aides during shifts per policies.
  • Monitor residents' condition and notify physician as needed.
  • Develop and review individualized care plans with the care team.
  • Complete accurate clinical documentation per policy.
  • Coordinate admissions, transfers, and discharges.
  • Participate in QA activities, staff education, and surveys.
  • Perform other duties as assigned by the Director of Nursing.

How you Will make a Difference
  • Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
  • Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
  • Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
  • Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
  • Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
  • Complete accurate and timely clinical documentation per facility policy and procedure.
  • Coordinate resident admissions, transfers, and discharges according to established procedures.
  • Participate in quality assurance activities, staff education, and regulatory surveys as needed.
  • Perform other duties as assigned by the Director of Nursing or designee.
What you Need to make a Difference
