Job summary

Westside Oaks Rehab is seeking a dedicated Registered Nurse (RN) to monitor, assist, and encourage patient progress in a community-based facility in Jacksonville, FL. The role involves coordinating with CNAs/PACs, administering medications, wound care, and documenting care progress.

12-hour shifts, with a sign-on bonus and walk-in interviews from 10 am to 3 pm. Responsibilities include discharge planning support, daily unit rounds, and ensuring compliance with regulatory requirements.

Qualifications

• Must hold a current and valid RN license.

Responsibilities

• Review, communicate and contribute to the day-to-day functions of the CNA and PCA.
• Provide medications by mouth, subcutaneous or intramuscular injections as ordered.
• Complete wound care as ordered to improve skin integrity.
• Make written and oral reports for continuity of care.
• Communicate with family members of residents as needed.
• Participate in center surveys (inspections).
• Assist in planning the nursing services portion of discharge plans.
• Admit, transfer, and discharge residents.
• Chart in electronic files using Point Click Care reflecting care and resident responses.
• Receive telephone orders from physicians on duty.
• Order prescribed medications, supplies, and equipment as necessary.
• Ensure narcotics handling complies with regulations.
• Develop work assignments.
• Make daily rounds to ensure resident safety.
• Meet with nursing personnel to improve services.
• Administer catheterizations, tube feedings, suction, dressings, and drainage bags.
• Attend in-service programs and continuing education.

Responsibilities:

• Review, communicate and contribute to the day-to-day functions of the Certified Nursing Assistants (CNA) and Personal Care Attendants (PCA)
• Provide medications by mouth, subcutaneous or intramuscular injections as ordered
• Complete wound care as ordered to improve skin integrity
• Make written and oral reports/recommendations concerning the activities of your shift for continuity of care
• Communicate with family members of residents as needed in the event of receiving new orders, a change in condition or a simple check-in to ensure the resident is safe, well, and content
• Participate and shine in center surveys (inspections)
• Assist in planning the nursing services portion of the resident’s discharge plans
• Admit, transfer, and discharge residents
• Complete accident/incident reports as necessary
• Chart in electronic files using Point Click Care in an informative and descriptive manner that reflects the care provided as well as residents' responses to care
• Receive telephone orders from physicians on duty
• Order prescribed medications, supplies, and equipment as necessary
• Ensure that narcotics are handled in accordance with federal, state, and local regulations
• Participate in associate performance evaluations and make recommendations to the Nurse Supervisor
• Develop work assignments
• Make daily rounds of your unit/shift to ensure resident safety is a Top Priority
• Meet with the nursing personnel on your shift a regularly to improve services
• Administer professional services such as catheterizations, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, etc
• Attend and participate in all center in-service programs as required
• Maintain professional competence through participation in continuing education programs, seminars, and training programs

Qualifications:

• MUST hold a current and valid RN license
