Signature HealthCARE of Hartford Rehab & Wellness Center is hiring RNs for night shifts. We offer newly increased wages and a generous sign-on bonus to join our dedicated team caring for residents across a 110-bed facility.

Join us to provide skilled nursing, coordinate care, and work with a supportive team. We value quality life initiatives and spiritual care, with opportunities for growth within a family-based organization.

Formación

• Current, active RN license in the state of practice.
• Minimum one (1) year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Proficiency with electronic health records (EHR) systems and Microsoft Office applications.

Responsabilidades

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living (ADLs) ensuring comfort, dignity, and safety.
• Supervise and direct nursing assistants and medication aides during the shift in accordance with policies.
• Monitor residents' status and report changes to physician and family.
• Develop and review individualized resident care plans with the interdisciplinary team.

We’re looking for dedicated nurses to join our night shift team and make a difference in the lives of our residents.

How you Will make a Difference

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.

What you Need to make a Difference

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Experience in a supervisory or charge nurse role.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.
