Job summary

Spring Creek Rehabilitation and Nursing Center is seeking a Full Time Registered Nurse (RN) for ALL SHIFTS at the Harrisburg, PA location. Must hold an active RN license in the state and be able to provide compassionate, age-appropriate resident care with a team-focused approach.

Responsibilities include assessments, care planning, medication administration, and supervising staff while ensuring compliance with laws and facility policies. Excellent orientation and career growth are offered.

Qualifications

• Must possess a current, unencumbered RN license in the state.
• Ability to work as a supportive team member and demonstrate teamwork.
• Knowledge of nursing practices, regulations, and facility guidelines.
• Ability to relate information about a resident’s condition to the team.

Responsibilities

• Evaluate performance and prepare evaluations for personnel.
• Perform nursing assessments and develop care plans with the care team.
• Plan and implement quality care programs and procedures.
• Set realistic patient goals and monitor outcomes to reduce rehospitalizations.
• Administer medications and treatments per physician orders.
• Delegate and supervise patient care duties to staff.
• Communicate patient information across shifts and with families.
• Work independently with flexibility and integrity.
• Assist in evacuation of residents during emergencies.

Requirements of the Registered Nurse (RN)

• Must possess a current, unencumbered, active license to practice as an RN in this state
• Must demonstrate knowledge and skills necessary to provide care appropriate to the age-related needs of the residentsserved.
• Must be a supportive team member, contribute to and be an example of teamwork and team concept.
• Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies orpersonnel, and the general public.
• Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines thatpertain to nursing care facilities
• Must possess leadership and supervisory ability and the willingness to work harmoniously with and supervise otherpersonnel.
• Must be able to relate information concerning a resident’s condition
• Must possess basic computer knowledge
• Must successfully complete a post offer, pre-employment criminal background check and physical / drug screen.

Responsibilities of the Registered Nurse (RN)

• Evaluate performance, initiate disciplinary actions, and prepare and complete performance evaluations for personnel.
• Perform nursing assessments and collaborate with the nursing team, other disciplines, patients, and families to develop effective plans of care.
• Plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures that are necessary for providing quality care
• Establish realistic, measurable short and long term patient goals with interventions put in place as needs arise to minimize rehospitalizations and continued evaluation of outcomes.
• Administer medications and performs treatments per physician orders.
• Delegate patient care responsibilities to staff and supervise them to ensure that tasks and interventions are implemented.
• Communicate patient information with assigned staff and between shifts.
• Function independently and have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies.
• Relate to and work with the ill, disabled, elderly, emotionally upset, and, at times, hostile people withinthe Facility.
• Assist in the evacuation of residents during emergency situation
