Job summary

Signature HealthCARE of Elizabethtown is hiring a Registered Nurse to provide direct nursing care, medication administration, wound care, and skilled nursing procedures. You will supervise nursing assistants, monitor residents, and coordinate admissions and discharges with the interdisciplinary team.

The role emphasizes safe, compassionate care, documentation accuracy, and regulatory compliance within a 112-bed facility offering short- and long-term care.

Qualifications

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Knowledge of Federal and State long-term care regulations.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.
• Effective verbal and written communication skills.

Responsibilities

• Provide direct nursing care to residents, including med administration and skilled procedures.
• Assist residents with ADLs, ensuring comfort, dignity, and safety.
• Supervise nursing assistants and medication aides during shifts.
• Monitor residents and notify physician of changes.
• Develop and review individualized care plans with the team.
• Coordinate admissions, transfers and discharges per procedures.
• Participate in QA, staff education and surveys as needed.
• Perform other duties as assigned by Director of Nursing.

How you Will make a difference

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.

What you Need to make a Difference

• Current, active RN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Experience in a supervisory or charge nurse role.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.
