Responsibilities
- Clinical Competence: Initiates, develops, implements, and revises the plan of care in collaboration with physicians and other healthcare professionals. Supervises care provided by home health aides and licensed practical/vocational nurses, provides instruction, and assigns tasks according to regulations. Performs supervisory visits as required.
- Documentation and Care Delivery: Provides high-quality clinical services within scope of practice and infection control standards, in accordance with the care plan, and in coordination with the healthcare team. Completes assessments (OASIS), medication reconciliation, and accurately documents patient visits per policy and payer requirements.
- Quality: Conducts nursing evaluations, ensures patients meet eligibility and medical necessity guidelines, develops the care plan with the physician, and submits accurate documentation. Communicates effectively with staff, physicians, patients, and families to ensure quality care and service excellence.
- Professionalism: Demonstrates punctuality, completes training, and participates actively in meetings and case conferences. Maintains a positive attitude, is open to learning, and complies with policies.
- Teamwork: Takes direction from leadership, completes assigned tasks timely, assists in orientation and training of new staff, and participates in quality improvement activities. Flexibility in scheduling, including weekends and holidays, is required.
- Utilization Management: Participates in the agency's utilization management model.
License Requirements
- Current RN licensure in the state of practice
- Current CPR certification
- Valid driver’s license, vehicle insurance, and access to transportation
